SEE ATTACHMENT 1: STATEMENT OF OBJECTIVES (SOO) SEE ATTACHMENT 2: TERMS AND CONDITIONS SEE ATTACHMENT 3: PAST PERFORMANE QUESTIONNAIRE SEE ATTACHMENT 4: REQUEST FOR PROPOSAL (i) "This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued."

(ix) The applicability of the provision at FAR clause 52.212-2, Evaluation - Commercial Items, and the specific evaluation criteria included in paragraph (a) of that provision is found in Attachment # 4 - Request For Proposal.
(x) Offerors shall include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer.
(xi) A statement that the FAR clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. Addenda Language to the clause applies, and is included in Attachment # 2 - Terms and Conditions.
(xii) The FAR clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition: FAR 52.223-18, Encouraging Contractor Policies to Ban Test Messaging while Driving (
